New Patients

Could you or your family member benefit from a medical genetics evaluation and

long-term care at the CHC?

Below are the steps to establish care at the CHC:

  1. If a healthcare provider (such as your primary care provider, ObGyn, etc.) is recommending you have a genetic evaluation, ask that healthcare provider to complete this referral form and send it to the CHC.

  2. If you would like to self refer, please call us at 260-593-0108

  3. Once we receive your referral, we will send you a new patient packet to complete so we can better assess your reason for referral and if the CHC is the right facility to address your healthcare needs.

  4. Once you return the new patient packet to us, we will review the information and contact you to confirm whether the CHC is a good fit for your needs and to schedule an appointment.

New patients to the CHC are typically first seen for an initial consult visit where we conduct a thorough review of your personal medical history and family history. Genetic testing may or may not be ordered at this visit. After the initial consult, we will request any records that we need for a full assessment, and then you will be scheduled for a visit with Dr. Ammous.

In an urgent situation or due to extenuating circumstances, we are sometimes able to combine the initial consult and assessment with Dr. Ammous in to a single visit. These visits tend to be lengthy and very involved, so most patients follow the process outlined above.

Insurance Information

We participate in the following types of insurance:

  • Medicare

  • MDWise

  • Managed Health Services (MHS)

  • Children with Special Healthcare Services

Although we do not accept private insurance, our staff will assist you in getting the information youneed to file a claim with your own insurance for the cost of your visit(s) at the CHC. We will bill private insurance plans for any genetic testing, laboratory work, etc. that is ordered as part of your evaluation. We also work closely with the Plain Church Group Ministry (PCGM) to provide care at the CHC at low cost for the Plain communities.
