A medical home is not a physical place, but a model of how health care should be. The Community Health Clinic recognizes how hard it is to navigate your way through the complex medical system, especially when you have a genetic condition. 'These conditions are often complicated and involve visits with multiple specialists.

Making and keeping track of appointments, remembering lab work and testing to be done, and compiling all of the information from many sources can be overwhelming. The CHC is in a unique position to help families dealing with complicated health care needs. We know how important it is to consider all the care needed to ensure the best possible health outcomes for our patients with genetic conditions. Appointments at the CHC may take 2 - 3 hours because we want to know our patients on a personal level and to understand their concerns.
