Plain Church Group Ministry

The Plain Church Group Ministry (PCGM) offers financial assistance for medical bills and provides a significant reduction in the cost of medical care for the Plain communities. This is done through bill negotiation and payment.

The PCGM does research on laboratory and diagnostic procedures, analyzes the quality of the service and offers counseling assistance for our patients.

Indiana Children’s Special Health Care Services

The Indiana Children's Special Health Care Services (ICSHCS) provides supplemental medical coverage to help families of children who have serious, chronic medical conditions, age birth to 21 years of age, who meet the program's financial and medical criteria, pay for treatment related to their child's condition. Our case management coordinator can help you apply for this program. Similar programs are available in most states, and we can connect you with the resources to access these programs if you live in a state other than Indiana.

Medicaid

Hoosier Healthwise is the Indiana health care program for children up to age 19 and pregnant women. This program is based on financial eligibility, and there are several options depending upon a household’s income. The Healthy Indiana Plan (HIP) is a plan for adults with lower incomes. Our case management coordinator can assist you with applying for Hoosier Healthwise for your child, or can connect you with programs such as Brightpoint, The Indiana Rural Health Association, United Health Services or other programs in your area that will help you apply for health insurance for you or your child.

Medicaid Waivers

Waivers are a part of the federally and state funded Medicaid program that provide reasonable and necessary medical care for persons meeting eligibility requirements. Waivers provide funding for children and adults with disabilities to receive support and services in the home and community rather than in an institutional setting. Indiana has two types of home and community - based waiver programs: medical and habilitative (for those with a developmental disability or intellectual disability or qualifying condition that results in functional limitations).

Applications for these waivers are done through the area Bureau of Developmental Disabilities Services (BDDS) offices, or through the local Area Agency on Aging. While eligibility for Medicaid is based upon household income, eligibility for Waiver programs does NOT take into consideration household income.

Newborn Screening Grant

Our Newborn Screening (NBS) Grant from the Indiana State Department of Health enables us to assist families whose infants and toddlers have failed the NBS test, and are being followed at the CHC for metabolic and genetic diseases. This coverage may include expenses that are not covered by your insurance and may include office visits, confirmatory testing, medication and supp lies, and is available until your child turns 4 years old.

When your child is referred to CHC as a result of a positive newborn screen, our NBS coordinator will discuss this option with you. You can also download the grant form here.

Private insurance

Although we do not take private insurance, our staff will assist you in getting the information you need to file a claim with your own insurance.

Patient Assistance Programs

Many testing and drug companies have programs that provide discounts for services and medication. At the CHC, we strive to help you find what you need at the most affordable cost.
