Referring Patients to the CHC
need a medical genetics evaluation for your patient?
Look no further than the CHC!
The CHc sees patients for a vast variety of indications including but not limited to:
Patients with a medical history suggestive of a possible genetic condition (ex.birth defects, developmental delays, intellectual disability, neurologic disease, hearing loss) who need an evaluation to determine if there is a genetic etiology for their symptoms
Patients with a known family history of a genetic disease (ex. hypertrophic cardiomyopathy, hereditary cancer predisposition syndrome, carrier status for a recessive genetic condition) who wish to know whether they have the same condition as their family members
Patients already diagnosed with a genetic condition who are requiring long-term follow-up (ex. Down syndrome, 22q deletion syndrome, inborn errors of metabolism such as PAH deficiency/PKU)
Please refer to the list of genetic conditions we have seen so far at the CHC. This list is rapidly growing! You may also refer to these guidelines for more information about when to refer a patient to a genetics clinic. When in doubt, call us and ask!
To Refer a Patient:
First, download and complete our referral form. Please fax the completed form to the CHC at 260-593-0116. Including any pertinent clinic notes, laboratory results, and/or previous genetic test results will assist with our review of the referral. We will then contact your patient with next steps/to schedule an appointment. Wait time for a standard visit is 2-3 weeks. These routine referrals follow the scheduling process outlined here.
If you are referring a patient who needs to be urgently evaluated (i.e. there would be an immediate impact to the patient’s medical management based on genetic test results), please indicate this on the referral form AND call us at 260-593-0108 so we are aware of the referral as soon as possible. We can typically see urgent patients within 2 weeks of the referral.
